Indications

Used to treat depression and depression-related symptoms.


Contraindications

  1. Patients who are allergic to the ingredients of this product.
  2. Patients who are currently using or have stopped taking monoamine oxidase inhibitors ( MAOIs ) within 2 weeks.
  3. Patients with severe hepatic impairment ( Child-Pugh classification C ).
  4. Patients with severe renal impairment ( GFR15mL/min ) or dialysis.

Dosage

  • Adult Usage :
    • Initial dose: 37.5 mg , once daily, after meals.
    • Maintenance dose: can be gradually increased to 75 mg or higher based on tolerance, with a maximum dose not exceeding 225 mg/ day.
    • Increment interval: at least 1 week.

Pharmacokinetics

  • Absorption : It is well absorbed after oral administration and food has no significant effect on absorption.
  • Metabolism : Mainly metabolized by the liver, metabolized by CYP2D6 and CYP3A4 .
  • Excretion : Mainly through urine.

Pharmacological Action

  • Mechanism of action : It exerts an antidepressant effect by inhibiting the reuptake of serotonin ( 5-HT ) and norepinephrine ( NE ), enhancing monoamine neurotransmission in the central nervous system.

Adverse Reactions

  1. Common adverse reactions :
    • Nausea, headache, dry mouth, insomnia, drowsiness, loss of appetite.
    • Palpitations, increased blood pressure, constipation, dry mouth.
  2. Serious adverse reactions :
    • Suicidal thoughts or behavior (especially in patients younger than 24 years).
    • Liver dysfunction, hyperglycemia, hypertensive crisis.
    • Psychiatric symptoms (eg, hallucinations, aggression).

Precautions

  1. Special Populations :
    • Pregnant and lactating women : Use with caution after a physician has evaluated the therapeutic benefit.
    • Elderly : Use under the guidance of a doctor and pay attention to dosage adjustment.
    • Children : Not applicable (patients under 18 years of age require careful evaluation).
  2. Driving and hazardous activities : The drug may affect concentration and reaction ability after taking it, so avoid driving or operating machinery.
  3. Drug interactions : Avoid co-administration with MAOIs , CYP2D6 inhibitors (such as paroxetine) and other drugs to avoid enhancing the efficacy or causing adverse reactions.
