2.      Indications

It blocks orexin receptors, regulates the sleep-wake cycle, and helps improve insomnia (such as difficulty falling asleep and difficulty maintaining sleep).

3.      Mechanism of Action

Vonorexant is an antagonist of orexin 1 (OX1) and orexin 2 (OX2) receptors, with binding affinities (Ki values) of 0.460 nmol/L and 0.374 nmol/L for human OX1 and OX2 receptors, respectively. By inhibiting the binding of the wakefulness-promoting neuropeptides orexin A and B to OX1 and OX2 receptors, it induces the transition from wakefulness to sleep.

4.      Dosage and Administration

Typical Use: For adults, usually 5mg once daily, orally, immediately before bedtime.

Dose Adjustment: The dose can be appropriately increased or decreased based on symptoms, but the daily dose should not exceed 10mg.

5.      Precautions

Use with caution in patients with narcolepsy or cataplexy, organic brain disorders, or respiratory dysfunction. Patients with mild to moderate hepatic impairment should be closely monitored after administration, and it is contraindicated in patients with severe hepatic impairment. Pregnant women should only use it if the benefit outweighs the risk, and breastfeeding women should weigh the benefits and risks before deciding whether to breastfeed.
