Indications
- Treatment of depression and depressive states.
- Pain caused by diabetic neuropathy.
- Pain caused by fibromyalgia, chronic low back pain, and osteoarthritis.
Contraindications
- Patients who are allergic to the ingredients of this product.
- Patients who are currently using or have stopped taking monoamine oxidase inhibitors ( MAOIs ) within 2 weeks.
- Patients with severe liver dysfunction.
- Patients with severe renal impairment.
- Patients with uncontrolled angle-closure glaucoma.
Dosage
- Depression and depressive states :
- Initial dose: 20 mg , once daily, after meals.
- Maintenance dose: can be gradually increased to 40mg or 60mg , the maximum dose should not exceed 60mg/ day.
- Diabetic neuropathy pain :
- Initial dose: 20 mg , once daily, after meals.
- Maintenance dose: can be gradually increased to 40mg or 60mg , the maximum dose should not exceed 60mg/ day.
- Fibromyalgia, chronic low back pain, osteoarthritis pain :
- Initial dose: 20 mg , once daily, after meals.
- Maintenance dose: usually 60 mg/ day.
Pharmacokinetics
- Absorption : It is well absorbed after oral administration. Food may affect the absorption rate, but there is no significant difference in the overall absorption amount.
- Metabolism : Mainly metabolized by CYP1A2 and CYP2D6 in the liver.
- Excretion : Mostly excreted in the form of metabolites through urine and feces.
Pharmacological Action
- Mechanism of action : It inhibits the reuptake of serotonin ( 5-HT ) and norepinephrine ( NE ), enhances monoamine neurotransmission in the central nervous system, and exerts antidepressant and analgesic effects.
- Pharmacological Action : Shows antidepressant, anxiolytic and analgesic effects in animal models.
Adverse Reactions
- Common adverse reactions :
- Nausea, dry mouth, constipation, headache, dizziness, drowsiness, insomnia.
- Thirst, loss of appetite, sexual dysfunction.
- Serious adverse reactions :
- Suicidal thoughts or behavior (especially in patients younger than 24 years).
- Liver dysfunction, hyperglycemia, hypertensive crisis.
- Psychiatric symptoms (eg, anxiety, aggression, hallucinations).
Precautions
- Special Populations :
- Pregnant and lactating women : Use with caution after a physician has evaluated the therapeutic benefit.
- Elderly : Use under the guidance of a doctor and pay attention to dosage adjustment.
- Children : Not applicable (patients under 18 years of age require careful evaluation).
- Driving and hazardous activities : The drug may affect concentration and reaction ability after taking it, so avoid driving or operating machinery.
- Drug interactions : Avoid co-administration with MAOIs , CYP2D6 inhibitors (such as paroxetine) and other drugs to avoid enhancing the efficacy or causing adverse reactions.
Packaging and storage
- Packaging : PTP packaging, 100 capsules per box ( 10 capsules × 10 ).
- Storage : Store in sealed container, avoid high temperature and humidity environment, and prevent children from accidentally ingesting.
